And self compiled kernel modules? Are they using the same modules folder?
My pi4 is running bookworm, so this is not problem.

The PI 5 will use the -2712 kernel by default, while the Pi 4B will use the -v8 kernel.

You can use kernel= in config.txt to make the PI 5 use the -v8 kernel, or you can build your modules for the -2712 kernel.

And additional question: will this image run on a zero2w?

It should do. The Zero 2W will run the -v8 kernel. Best if it is a "lite" images. The Zero 2W isn't great for desktop use.
