HATs and other add-ons • RPi 3B+ V1.4 PMIC Problem

Hello everybody

I reach out to you to first to understand and afterwards to solve the following problem:
We have a hat designed for RPi 3B/3B+ that provides 5v power via GIPO 2 & 4 and GND on GPIO 6 to the RPI. We have been using this hat for the last 6 (six) years with no field problem. The hat is designed by us inhouse.
When we purchased a new batch of RPi 3B+ we discovered that the new boards, v1.4, have a redesigned PS and we had to update the Buster to version 5.10.103 in order to be able to boot, and retain the Buster OS variant for our software. Done successfully.
Now when using the v1.4 RPi3b+ with the above hat the RPI has a very short life. Either the PMIC got shorted instantly - no 3v3 on pin 1 - or it can fail randomly after a week or two of field usage.
The voltage supplied by the hat is 5.172V with no load. Pin 1, 3v3, is not connected on our PCB. On our hat PS stage we are using TPS61235P
Customer support of Farnell was not helpful at all, maybe an AI bot got over the support department.

Any thoughts from the experts?
