except you state that without an SD card, the diagnostic display appears.

So, i'd still say cabling. if you are using an "adapter" i'd call that suspect.
i only use the "correct" cable (micro-hdmi to hdmi) on all my setups.
hard-coding the display resolution in cmdline.txt usually works. i'm surprised is doesn't work in your case?

but clearly the PI is not getting the EDID data. (as evidenced by EDID=none)

for now, i'd just try 1 monitor, on the first port, and the resolution set in cmdline.txt
