I don't believe you can conclude that.Seems to "program" fine but isnt keeping it in the flash.
It's either not writing to Flash and you are seeing what was previously written to it or corrupted data when read, or it's writing but what is written is corrupted on write or read.
I would suggest crafting a Run from RAM program which will prove it can execute code from RAM. Then enhance that to show what's in Flash so you can tell it's not randomly changing each time and can then tell if it's being written to and changed by 'picotool'. Then further enhance that to test writing to Flash works and you can read back what was written.
It is worth testing with another IC because that might provide some insight.
